The Chronic Disease Management team in Sunrise Health Region is excited to announce the new Cardiac Rehabilitation program for residents of our region.
The Cardiac Rehabilitation program is a clinically supervised exercise and education program aimed at assisting people in recovery from a heart event and helping them reduce their risk of future events.

Who is it for?
Cardiac Rehab can benefit you if you have experienced:
- Heart Attack
- Chest pain/angina
- Bypass surgery
- Heart valve surgery
- Any other heart condition
- Risk factors for Cardiovascular Disease (high blood pressure, diabetes, high cholesterol, weight gain)
How do I sign up?
Referrals for the exercise program are accepted from Health Care Providers. (Self Referrals are also accepted.)
No referral is required to participate in the education program. Simply contact us at the phone number below to sign up for an education class.
What does it cost?
Education sessions are free of charge.
The Exercise program is free of charge in Melville.
The Exercise program costs $2.00 per session in Yorkton.
Why should I join?
Cardiac Rehab provides a supervised environment to safely increase your activity/exercise levels.
During exercise you will be monitored and supervised by Registered Nurses and an Exercise Therapist who have special training in Cardiac care and Emergency care.

Where do the programs take place?
Education classes are held throughout the Sunrise Health Region.
Education on diet, medications, stress management, exercise, and risk factors is offered over the coure of five 1.5 hour sessions.
At this time, the exercise portion of the program is being offered in Yorkton at the Gallagher Center and at the new Horizon Credit Union Center in Melville.
Exercise takes place twice a week for 6-8 weeks
Each exercise session lasts 1.5 hours
Exercise programs are individually designed around your medical history and personal goals.
